PROACTIVE FORM TEACHERS CAN USE TO REQUEST LESS THAN ANNUAL EVALUATIONS--


Fewer Than Annual Appraisals Teacher Authorization Document

Pursuant to changes in Board Policy DNA(LOCAL) amended in April 2006, teachers eligible for fewer than annual appraisals shall be appraised once every three years.

Please consider this document as my authorization to you as my teacher supervisor or appraiser to appraise me on a less than annual basis under the guidelines listed in amended policy DNA(LOCAL).
